Lending Agreement

In order to become a user of the Enlace Lending Library, you must print, sign and return this form to: University of Connecticut, Center for Latin American and Caribbean Studies, 843 Bolton Road, Unit-1161, Storrs, CT 06269-1161. For future orders we require only that you notify us of any changes of address or telephone.

I, ___________________________________, agree to return all items borrowed from the Lending Library in the same condition I borrowed them and by the date specified to me in the accompanying letter.  I agree to package the items securely in a box or padded envelope and to insure for $150.00.  I will keep all insurance or UPS receipts for at least 90 days as proof of shipment. If an item is damaged or lost before I mail it, I agree to help the Center for Latin American and Caribbean Studies to replace it in a manner to be agreed upon by myself and the Education Outreach Assistant.
